APPOINTMENT SCHEDULING
Thank you for your interest in making an appointment. I am accepting new patients for in-person or telehealth visits. Please review the Practice Policies (which includes service pricing) and FAQ sections of the website to ensure that my practice will work for you. If you agree to the practice policies and would like to schedule an appointment, please email or call the office. Be sure to include your name, preferred way to be contacted, and share how you were referred to the practice.
After your appointment is scheduled, you will receive an email with instructions to help make the most of your session that should be completed the week before your appointment. This will include completing an intake assessment survey, which provides evidence-based data to help enhance your care, as well as completing some forms and entering medical information on the portal as part of the “check-in” process. Please note that system scheduling emails will come from noreply@patient-message.com. It is recommended that you mark this email as safe or add it to your contacts to ensure you receive these messages. Please also note that you cannot reply to those emails and to contact the office directly if you need any assistance. Finally, if you are scheduled for a telehealth appointment, you may go to the telemedicine section of the website where you can learn more about preparing for and connecting to your virtual session.
